Individuals who are enrolled in Medicaid may qualify for services under this program.
If you are pregnant or up to 12 months after giving birth, you may qualify for enhanced services, including additional nutrition support.
You may also qualify if you have certain health conditions or life circumstances that make it difficult to access healthy food or maintain your well-being. This may include chronic health conditions, high-risk medical needs, or other challenges affecting your daily living.
If you live with someone who qualifies (such as a pregnant individual or someone with additional needs), other members of the household (such as a spouse or children under 18) may also be eligible for support.

Services are provided for each approved member of the household, based on eligibility and program guidelines.
